Patient Complaint

If you have a grievance or complaint, please email QASTAFF@lcmcwmh.com or call (928) 289-6323 and leave a brief message stating your name, the patient's name and a contact telephone number. A representative of Little Colorado Medical Center will contact within 3 business days.

As a patient/patient representative it is your right to file a complaint with the Arizona Department of Health Service if you feel that your grievance is not addressed appropriately or in a timely manner.
